On Thursday Jana Litsey, deputy general counsel of Bank of America Corp. (BoA), signed her name to the largest settlement ever reached between the federal government and a company. Twice.

The settlement’s signature pages had spots for two bank officials to sign the document, along with a slew of government lawyers headed by associate U.S. attorney general Tony West. Apparently someone was expecting a bank officer to join in the deal, but Litsey filled in both spots.
