Board of Standards & Appeals: 250 Broadway, New York, N.Y., 10007; Civilian Complaint Review Board: 100 Church St., 4th floor, 10007; Department of Probation: 100 Centre St., Room 1031B, 10013, calls can be made to 212-442-4550; Department of Transportation: 100 Church St., 4th floor, 10007; Human Resources Administration/Department of Social Services: 250 Church St., 10013, calls can be made to 212-274-5828/5928; Office of Administrative Trials and Hearings, 66 John St., 10th floor, 10038, as soon as service is established, the contact number will be 212-361-1400; Office of Labor Relations: 250 Broadway, 28th floor, 10007, calls can be made to 212-602-6014; Taxi and Limousine Commission: 32-02 Queens Blvd., Long Island City, N.Y., 11101.
