The Supreme Court’s July 19, 2012 Omnibus Rule Amendment Order inter alia deleted Appendix XXI (“Bail Program Registration Form”) effective September 2, 2012.
In that Order, the caption of deleted Appendix XXI was shown erroneously as “Insurance Producer/Limited Insurance Representative Form (R. 1:13-3(d)),” rather than the correct “Bail Program Registration Form (R. 1:13-3(d)).” Similarly, the language added to the source note for deleted Appendix XXI should have read “Appendix XXI (‘Bail Program Registration Form (R. 1:13-3(d))’) deleted July 19, 2012 to be effective September 4, 2012″ rather than “Appendix XXI (‘Insurance Producer/Limited Insurance Representative Registration Form (R. 1:13-3(d))’) deleted July 19, 2012 to be effective September 4, 2012.”
