Here are a few of the 51 questions (substantially abridged) presented in the 13-page confidential questionnaire that all New York state judicial candidates must submit. � Are you able to handle emergency applications, cope with media scrutiny, issue quick decisions, deal with fractious litigants, and otherwise respond to stressful situations? � Have you ever been the subject of a claim about discrimination or harassment on the grounds of age, race, creed, color, gender, sexual orientation, religion, national origin, disability, marital status, socioeconomic status, alienage or citizenship status? � Are you now, or have you ever been, the subject of any formal complaint or charge filed with any disciplinary committee? � Have you ever been summoned, arrested or charged with any offense or crime? � Are there any unsatisfied judgments, tax warrants, tax liens or mechanics’ liens outstanding against you? � Has any governmental agency or grand jury initiated or completed an investigation of you or of any law firm, business, partnership [or] joint venture with which you were affiliated? � Attach a statement specifying the types and number of each type of trials you have conducted in the past 10 years; the number of the cases which ended in a verdict or judgment or ruling by trier of fact; the number of cases which were tried before a jury; the number and types of appeals briefed in the past 10 years; the number and types of dispositive motions you have litigated in the past 10 years. � Submit a list of the past 10 trials, dispositive motions or appeals in which you have activity and substantially participated, including the title of the case, the index, docket or indictment number, a concise description of the nature of the case; the name, address and telephone number of each adversary and co-counsel, and the name(s) of the judge(s) who presided at trial. � Submit a list of the name, court and telephone number of the last 10 judges before whom you appeared in the last three years.
