This notice is to advise the bar of revisions to Side 2 of the Civil Case Information Statement (Appendix XII-B to the Rules of Court). The revisions, which are to the reverse side of the form only, are effective April 1, 2005. The revisions include the following: a. elimination of Rezulin and Propulsid as numbered mass tort case types under Track 4, since those cases have been concluded; b. elimination of Repetitive Stress Syndrome cases under Track 4, since those cases should instead be included under Personal Injury in Track 2; and c. under Track 1, clarification (by adding a parenthetical) that Real Property cases do not include cases in other numbered case types, e.g., tenancy, construction.
